The concern
The patient came to our Fort Lauderdale clinic with a single, very common concern: he looked tired even when he wasn't. The hollows under his eyes had become more pronounced over time, and the resulting shadow across the under-eye area was making him look fatigued in photos and in the mirror. The hollowing itself was mild rather than severe, which actually opens up more conservative treatment options. The consultation
Johana assessed the patient's tear trough hollowing, lower lid skin quality, and the transition from lid to cheek. The clinical decision was to use 0.40ml of Restylane-L, placed deep on the bone using a blunt cannula. Two technical choices drove the result. First, the cannula. The under-eye area is highly vascular, and a blunt-tipped cannula significantly reduces both bruising risk and the risk of injecting into a blood vessel compared with a sharp needle. Second, the depth. Placing filler deep on the bone reduces the chance of two common under-eye complications: the Tyndall effect, which is a bluish tint that can appear when filler sits too superficially, and a lumpy or uneven appearance that can develop when product isn't placed precisely. The dose was deliberately conservative. Tear trough fillers are hydrophilic, meaning they attract water over time. Correcting the hollow completely would risk over-filling as the product settled. The goal was to lessen the hollow, not eliminate it.
The treatment
The appointment took around 25 minutes from start to finish, with numbing applied before injection. The cannula was introduced through a small entry point and 0.40ml of Restylane-L was placed in small amounts deep against the bone. The patient left the clinic the same day with mild swelling at the entry points, which resolved within a few days.
Results at two weeks
The two-week photograph shows softened under-eye hollows and a noticeably brighter, more rested under-eye area. The shadow that previously sat beneath the lower lid has lifted, and the patient's overall expression reads fresher and less fatigued. There is no puffiness, no lumpiness, no bluish tint, and no obvious sign of treatment. The conservative dosing achieved its goal of correction without overfill.
