The concern

The patient came to our SoHo clinic initially concerned with general facial ageing and signs of sagging across her face. She had noticed darker, more hollow under-eyes, and a softening of structure across her midface and lower face that hadn't responded to skincare alone. Looking tired even when she wasn't was one of the things that bothered her most — and the under-eye was where it showed first. At consultation, Victoria discussed a broader plan with the patient: filler for the nasolabial folds and marionette lines, Sculptra to support overall structure over time, and immediate treatment to the under-eye area as a starting point. The patient's first appointment focused on under-eye filler and anti-wrinkle to her upper face and neck — the foundation of an ongoing treatment journey.

The consultation

Victoria assessed the patient's under-eye area carefully. The clinical decision required particular consideration: the patient has a fat pad under her eye, which makes the under-eye area more complex to treat. Filling the tear trough too aggressively in a patient with a fat pad emphasises the pad itself — trading one problem (a hollow) for another (visible puffiness). The plan was deliberately conservative: 0.8cc of Restylane-L split evenly as 0.4cc per side, with a hybrid technique. Some product placed in the true tear trough using a blunt cannula to minimise bruising in a highly vascular area, and the remainder placed by needle directly to the periosteum (the surface of the bone) at the lateral portion of the under-eye. The goal was correction without over-filling — a softer hollow, not a fuller under-eye.

The treatment

The appointment took around 30 minutes from start to finish, with numbing applied before injection. The cannula was introduced through a small entry point at the lateral cheek and product was placed slowly and conservatively into the deep tear trough. Needle injections were then placed at the lateral periosteum to support the outer portion of the under-eye. The patient left the clinic the same day with mild swelling and possible minor bruising at the injection sites — both expected, both typically resolved within several days.

Results at two weeks

The two-week photograph shows softened tear trough hollows, a brighter overall under-eye area, and a more rested appearance. The conservative dosing achieved its goal: the hollow has been corrected without emphasising the fat pad above it. The under-eye reads smoother and less shadowed, without any added puffiness or "shelf" effect. The patient retains her natural eye shape and resting expression — what's changed is the brightness of the under-eye area, not its structure.
