Best Botox & Filler Treatments on the Upper West Side

If you live on the Upper West Side, your standards for medical aesthetics are probably already pretty specific.

You don’t want overdone. You don’t want a big production. You want a clinician who knows what they’re doing, in a clinic that doesn’t feel like a sales floor, with results that don’t announce themselves at school drop-off.

This guide walks through the Botox and filler treatments that consistently deliver the best results for UWS patients — what each one does, who it’s for, and how to think about a longer-term plan. It’s written by Sisu Clinic, where every injection is performed by a licensed nurse practitioner under medical leadership.

How to think about Botox vs filler

Before getting into specific treatments, the most useful frame is this: Botox relaxes, filler restores.

Botox is for lines created by movement — the horizontal forehead lines, the vertical ‘11s’ between your brows, the crow’s feet at the corners of your eyes. It works by gently relaxing the muscles that fold the skin into those lines. It is not adding anything; it is calming muscle activity.

Filler is for areas that have lost volume or never had the structure you want — under-eye hollows, flatter cheeks, less defined chin or jawline, thinner lips. Hyaluronic acid filler is gel that sits within the tissue and gives shape, support and contour.

Most UWS patients in their 30s and 40s end up using both, but in very different doses than people imagine. The right plan is small amounts, consistently, over time. Not a single dramatic intervention.

The best Botox treatments on the Upper West Side

1. Forehead, frown lines and crow’s feet (the upper-face trio)

The most requested, and arguably the most life-changing, Botox treatment for UWS patients in their late 20s, 30s and 40s. Treating all three areas together — the forehead, the 11s, and crow’s feet — keeps the upper face balanced. (If you only relax your 11s and not your forehead, you can end up with a heavier brow look.)

Why it’s so popular here: most UWS patients are reading-light, screen-heavy professionals. Frowning at a laptop is the single biggest contributor to deeper 11s. Soft, consistent treatment in this trio is the foundation of a long-term natural-looking plan.

What ‘the best’ looks like: forehead movement preserved, 11s softened, eyes still smile. You should look like you slept eight hours, not like you had work done.

2. Lip flip

A lip flip is a few units of Botox along the upper lip border. The muscle that pulls your upper lip down softens, and the lip rolls outward subtly, creating the appearance of a fuller lip without any filler. It’s our most popular ‘gateway’ treatment for patients who want softness around the mouth but aren’t ready for filler.

Particularly suited to UWS patients in their 30s who feel their upper lip has thinned slightly with age. Pairs beautifully with a small amount of lip filler, but works on its own too. Lasts about 6 to 8 weeks, which is shorter than facial Botox because the muscle is small and active.

3. Masseter (jaw slimming) Botox

Botox into the masseter muscles slims the lower face, softens jaw squareness, and can also relieve jaw clenching, teeth grinding and the headaches that come with both. Results build over 4 to 6 weeks and last around 4 to 6 months.

This one is enormously popular with UWS patients who clench at night (a city-stress signature), grind their teeth, or feel their lower face has become heavier with age. It also has the side benefit of giving the jawline a softer, slightly more lifted look in photos.

4. Preventative Botox

Lower-dose Botox started before deeper static lines have set in. This isn’t a separate product; it’s a different philosophy. The goal isn’t to fix what’s already there but to slow the formation of the next decade of lines. Often booked by UWS patients in their late 20s and 30s as small, twice- or three-times-yearly treatments.

There’s a longer post just on this topic in this blog series — it’s worth reading before you decide if it’s right for you.

5. Chin (mentalis) and lower-face smoothing

Tiny amounts of Botox into the mentalis muscle reduce the dimpling that can develop on the chin with age. Often combined with subtle lip flip and a touch of jawline filler for a smoother, more refined lower third.

6. Hyperhidrosis (underarm sweating)

Botox is FDA-approved for severe underarm sweating, with a single treatment lasting 6 to 9 months. Quietly one of our most-requested non-cosmetic treatments in NYC summer.

The best filler treatments on the Upper West Side

1. Lip filler — soft, structural, never overdone

Most lip filler problems are not lip filler problems; they are dose problems. The best lip filler is built incrementally: a small amount placed accurately to support the lip’s natural shape, then evaluated.

At Sisu UWS, most first-time lip filler patients have 0.5ml or 1ml. We do not chase a trend, we do not build out the lip beyond your face, and we will tell you the truth if we think you’re asking for something that won’t suit you. Results last 6 to 12 months.

2. Cheek filler — the structural foundation

Cheek filler restores the volume that quietly disappears across your 30s and 40s, lifting the midface and softening nasolabial folds without ever touching the nasolabial folds directly. The right cheek treatment is a structural treatment, not a fullness treatment. Done correctly, it makes you look rested and refreshed in photos rather than puffier.

This is one of the most under-prescribed treatments by less-experienced injectors and one we use routinely. Results last 12 to 18 months.

3. Under-eye (tear trough) filler

The single most-requested UWS treatment for patients who feel they look tired in photos. Carefully placed hyaluronic acid filler in the tear trough softens hollowing and the dark shadow that hollowing creates.

This treatment is technique-sensitive. Placed wrongly, it swells, holds water, or migrates. We only treat under eyes for appropriate candidates, and only with our most experienced injectors. Sometimes the right answer is no, and we’ll say so.

4. Chin and jawline filler

Subtle chin projection and jawline definition are some of the highest-impact, lowest-detection filler treatments available. They balance the lower face, sharpen photographs, and often reduce the appearance of jowls without addressing the jowls directly.

Particularly effective for UWS patients in their 40s and 50s who feel their face has lost ‘shape’.

How to choose an injector on the Upper West Side

There are dozens of clinics offering Botox and filler in this neighbourhood. A short, practical checklist:

  • Is the injector a licensed medical professional (NP, PA, MD, RN under medical direction)? It is a fair question to ask before booking.
  • Is the clinic doctor-founded or under medical oversight? This matters when something doesn’t go to plan.
  • Is the consultation free, and unrushed? A 10-minute consultation followed by an immediate treatment is not a consultation.
  • Is there a follow-up policy? Two-week reviews and complimentary adjustments are a sign of a clinic that owns its results.
  • Can the injector show you photos of natural-looking results, not just dramatic before-and-afters? The natural ones are harder to fake.
  • Will they ever say no to a treatment? A clinic that always says yes is selling, not consulting.

Building a long-term plan, not chasing a single treatment

The patients who look the most natural across their 30s, 40s, 50s and 60s are not the ones who do dramatic interventions. They are the ones who treat consistently, conservatively and early.

A typical Sisu UWS plan for a patient in their mid-30s might look like this: Botox three times a year (forehead, 11s, crow’s feet), a single course of Profhilo annually, and a small refresh of lip or under-eye filler every 9 to 12 months as needed. That’s it. That’s the whole plan.

It is not glamorous. It’s also why people ask, with genuine confusion, why they look so well.
